Case Report: Complex Extraction and Graft with 3 year post-op fully restored by Dr. Rick Sullivan

Long time patient reported with large swelling on the buccal of 3 in January 2021. CBCT revealed large apical radiolucency with missing buccal plate. After IV sedation, tooth was sectioned and removed without incident. This was done using the proximal spade technique that we teach in our ext/graft course.

Socket was cleaned, degranulated and filled with ‘sticky bone.’ A resorbable collagen membrane was placed that covered the buccal defect and wrapped over the ridge and under palatal tissue and was secured in place with an apical screw and periosteal sutures. A L-PRF ‘slug’ was placed and all was secured with another round of sutures.

After 4 months, a conelog implant was placed and the implant was scanned using the conelog scanpost per the technique so wonderfully taught by Kevin Fryer. After 4 months, the implant was exposed and the final restoration placed.

Final bitewing was taken today at recall hygiene visit showing excellent crestal bone quality at the 3 year mark.
